- Current driver license number and driver license history for the past 3 years

- Employment history up to 10 years

- History of traffic accidents, violations and/or convictions from the last 3 years (including DUI or reckless driving conviction and license suspension)

TO QUALIFY WITH BALL INTERMODAL

YOU MUST MEET THE FOLLOWING CRITERIA

- Must present a clean well groomed appearance

- Must possess only one (1) valid drivers license, SS card, and immigration document as may be required

- Must have acceptable references from previous employers

- Must not falsify application

- Must have no conviction for DWI or DUI within the past ten (10) years

- Must not have been convicted of reckless or careless driving last ten (10) years

- No license suspensions or revocations for driving conduct

- No convictions for alcohol or drug related conduct

- No more than two (2) convictions for moving violations within the past three (3) years

- No major preventable DOT recordable accidents within the past three (3) years

- Must pass physical examination, drug testing, and company road test

- A minimum of three (3) years verifiable recent driving experience

- Must read and write the English language well enough to fulfill the requirements of the position applied for, per 391.11 (b) 2

- No more than one (1) serious violation (as defined by part 383.5) in the past year

- No more than five (5) jobs in the past three (3) years

- An applicant that submits a "diluted" urine specimen must submit to "retesting" within four (4) hrs.of notification of test results

- Applicants who have tested positive for alcohol or controlled substance within the previous ten (10) years or refused to submit to such testing will not be hired
